Parent/Guardian Acknowledgment

I, the undersigned parent or legal guardian of the child named below:

1. Consent to Participate
   • Give permission for my child to take part in the ZIMAA Kids’ Area activities.
2. Supervision Requirement
   • Understand that I must remain with and supervise my child at all times.
   • If I choose to leave my child unattended, I do so at my own risk, and ZIMAA is not responsible for any incident or injury that may occur.
3. Medical Disclosure
   • Agree to provide details of any relevant medical conditions, injuries, or allergies below.
   • I understand failure to disclose may increase risks during participation.
